Case 21.1  Very Early Onset IBD

A 5-year-old female with history of slow weight gain presents with intermittent bloody diarrhea and periumbilical abdominal pain for the past 1 month. There is no fever, her weight < 5th %, and height is 25th %. The abdomen is soft, non-tender, non-distended with normal bowel sounds, normal perianal exam, and normal joint and skin exam.

Birth history: Normal full-term infant, non-consanguineous parents Was formula fed but did develop blood streaked stool at 4 months of age which resolved after change of formula. Has had allergy evaluation completed previously which was negative. Currently no dairy in her diet.

Family history: negative for IBD or autoimmune diseases.
