You initiate vedolizumab 300 mg IV at weeks 0, 2, and 6, followed by 300 mg IV every 8 weeks.  The patient weans from prednisone successfully, and 6 months later a colonoscopy demonstrates Mayo 0 disease throughout the colon, with patchy chronic active colitis histologically.
