Case 9.2

He is presumed to have left-sided ulcerative colitis with a cecal patch and started on budesonide, oral mesalamine, and mesalamine enema to induce and maintain remission. After two weeks of therapy, he has increasing urgency, more frequent bloody bowel movements (8 per day), and worsening abdominal pain.  He presents to the emergency room for admission.

Initial evaluation demonstrates:

Temp: 36.5 | HR 74 | BP 113/52

  • Hgb 13
  • Albumin 2.9
  • CRP 21, ESR 23

The decision is made to admit him to the hospital for acute severe ulcerative colitis.
